Understanding Haglund's Deformity and Tendon Tension
Haglund's deformity is often recognized by the prominent bump that develops on the back of the heel. Many people focus on the visible enlargement because it can become irritated by footwear and cause discomfort during walking or exercise. However, the irritation and enlargement are not caused by footwear, only noticed with subsequent rubbing on the shoe. The cause is calf and achilles tightness.
The Achilles tendon attaches directly to the back of the heel, creating a connection between the calf muscles and the area where symptoms often develop. Understanding that relationship can provide valuable insight into why irritation occurs and how restoring mobility using Active Release Techniques (ART) as an assisted stretching technique, eccentric exercises and Active Isolated Stretching (AIS) may become part of resolving the tightness and reducing the bump on the heel.
What Is Haglund's Deformity?

Haglund's deformity refers to a bony enlargement that develops near the back of the heel bone. The prominence can create friction against shoes and surrounding soft tissues, particularly during activities that involve frequent walking, running, or climbing stairs.
People with Haglund's deformity often notice tenderness near the back of the heel, swelling around the area, or discomfort when wearing shoes with a rigid heel counter. Because the enlargement sits close to the Achilles tendon attachment, irritation may affect nearby structures as well.
Why Achilles Flexibility Matters
The Achilles tendon forms from the gastrocnemius and soleus muscles in the calf. Together, these muscles generate the force needed to push the body forward during walking, running, and jumping.
Every step places load through the Achilles tendon, and when the gastrocnemius and soleus lose flexibility, the tendon experiences greater tension throughout daily movement. That increased pull transfers force directly into the heel where the tendon attaches.
Over time, restricted calf muscles can create an environment where the attachment area experiences repeated stress during normal activity. As a result, the Achilles tendon and surrounding tissues may struggle to move as efficiently as they should. The body tries to reinforce the attachment site on a daily basis, resulting in the thickening of the calcaneus (Haglund’s deformity).
The Problem Isn't Just the Heel Bone
Many treatment approaches focus entirely on the bump itself because it is the most visible part of the condition. While the enlargement may contribute to irritation, the forces acting on the area are the cause that started well before the bump and the unpleasant sensation began.
A tight Achilles complex continuously influences the mechanics of the foot and ankle. Limited flexibility can alter how the ankle moves during walking and reduce the amount of motion available as the body progresses over the foot. Those changes affect the structures attached to the heel with every step. Looking beyond the bony prominence allows treatment to address the tissues influencing movement rather than focusing solely on the visible symptom.
Restoring Length to the Achilles Complex

Active Release Techniques (ART) can help address soft tissue restrictions that limit normal movement within the calf muscles. Active Isolated Stretching (AIS) works to improve flexibility and restore available motion throughout the ankle and lower leg. People may also learn specific stretching techniques to perform at home, allowing them to continue improving tissue mobility between visits. Improving flexibility within the Achilles complex can help support healthier movement patterns and reduce unnecessary tension acting on the heel attachment area.
Warning, improper stretching and tugging on the insertion point can further worsen the condition. Taping with a rigid cloth tape (Leukotape), utilizing Low-Dye technique will help to decrease daily irritation. Flexible tape does not provide the support needed to reinforce tendinous fibers into the bone.
Looking Beyond the Bump
The visible enlargement associated with Haglund's deformity often receives the most attention, but the tissues connected to the heel also influence how the area functions. Addressing flexibility within the Achilles complex may help improve movement and support long-term foot and ankle health. Allowing the body to properly respond with inflammation will allow proper cells to reach the site of the vulnerability. Ice slows blood flow and will delay the proper healing after tissue micro-trauma (such as exercise).
