Understanding the Role of Toe Extensor Muscles

A hammertoe occurs when the middle joint of a toe bends downward, creating the curled appearance many people recognize. While footwear often gets blamed for the condition, shoes do not lengthen or shorten muscles and tendons. To understand why a hammertoe develops, it helps to look at the structures responsible for controlling toe movement. Let's take a closer look at the role of the extensor digitorum longus and extensor digitorum brevis and explore how stretching techniques, like Active Isolated Stretching (AIS)  may help restore motion.


The Muscles That Control the Toes

The extensor digitorum longus and extensor digitorum brevis help lift and control the lesser toes during movement. Both muscles divide into four tendons that travel toward the second, third, fourth, and fifth toes. Every step needs these structures to coordinate toe movement as the foot moves through the walking cycle. They help position the toes during ground contact and guide movement as the body transitions from one step to the next. Because these muscles stay active throughout daily movement, they depend on normal flexibility and tissue length to function efficiently.


What Happens When These Tissues Shorten?

The body adapts to the demands placed upon it over time. When the extensor digitorum longus or extensor digitorum brevis lose normal length, the tendons connected to the toes can create a persistent pulling force across the joints.


As that restriction develops, the middle toe joint may spend more time in a flexed position. Instead of moving freely through its full range of motion, the toe gradually assumes a bent posture that becomes increasingly noticeable. The change often develops slowly, making it easy to overlook during its early stages.


Restoring Motion Through Stretching

Improving flexibility within the affected tissues requires a different approach. Assisted stretching and manual stretching techniques focus on restoring length and mobility within the extensor digitorum longus and extensor digitorum brevis.


Active Isolated Stretching (AIS) can help improve flexibility while encouraging healthier movement patterns throughout the foot. Learning specific stretches to perform at home also allows individuals to continue working on mobility between visits, creating more opportunities to maintain progress. Rather than focusing solely on the position of the toe, stretching addresses the movement capacity of the tissues influencing that position.


Why Footwear Does Not Change Muscle Length

Many people try wider shoes, inserts, toe sleeves, or orthotics after noticing a hammertoe. These options may improve comfort by reducing pressure against the affected toe and creating additional space within the shoe. However, comfort measures do not lengthen shortened muscles or tendons. While they may help accommodate the position of the toe, they do not directly improve mobility within the extensor digitorum longus or extensor digitorum brevis.
