Understanding Chronic Exertional Compartment Syndrome

Chronic exertional compartment syndrome develops when pressure rises within one or more muscle compartments of the lower leg during exercise. The condition often affects runners, hikers, military personnel, and other active individuals whose symptoms appear during activity and improve with rest. Learning why that pressure develops can also help explain how Active Release Techniques (ART), Active Isolated Stretching (AIS) and eccentric weight training exercises focus on restoring tissue mobility and normal movement.
What Is Compartment Syndrome?
The lower leg contains several muscle compartments surrounded by a dense layer of connective tissue called fascia. Each compartment houses muscles, nerves, and blood vessels that work together during walking, running, and other physical activities.
With chronic exertional compartment syndrome, pressure increases inside one of these enclosed spaces during exercise. As that pressure rises, the muscles and surrounding structures have less room to function normally. Many people experience aching, tightness, cramping, or a feeling of fullness that gradually worsens during activity before easing with rest.
Restoring Mobility Within the Lower Leg
Active Release Techniques (ART) help address soft tissue restrictions that may limit normal movement between muscles and fascia. Active Isolated Stretching (AIS) works to improve flexibility and muscle length, allowing the lower leg to move through a healthier range of motion. Eccentric weight training retrains the muscle to be more efficient at lengthening and staying in a more relaxed state during movement. People can learn specific home stretches to help maintain mobility between visits.
Rather than simply addressing symptoms after activity, the focus should be on improving movement quality within the tissues that repeatedly experience stress during exercise. By improving the way these tissues move together, people may experience better lower leg function during repetitive activities such as running, hiking, and other forms of exercise.
Why Exercise Can Trigger Symptoms

During physical activity, the muscles naturally increase in size as blood flow rises to meet greater demand. Because the fascia surrounding each compartment has limited ability to expand, that increase in muscle volume also raises pressure within the compartment.Â
In people with chronic exertional compartment syndrome, the pressure may continue rising during exercise, contributing to the aching, tightness, or cramping that develops with activity.
Who Commonly Experiences Chronic Exertional Compartment Syndrome?
This condition most often affects people who perform repetitive lower leg activities, especially those involving running or jumping. Common examples include:
Distance runners
Trail runners
Military recruits
Soccer players
Basketball players
Hikers
Symptoms often develop at a fairly predictable point during exercise and improve after activity stops, making the pattern itself an important part of recognizing the condition.
